Job Description

Step into a pivotal role supporting patient care by conducting home health risk assessments for Medicare Advantage members across Clay County, including Brazil, Clay City, and Knightsville. This per diem position is ideal for Nurse Practitioners who thrive in independent, patient-centered environments and value flexible scheduling options.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Design your ideal work schedule
  • Meet a minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments averaging 45 minutes per visit
  • Review and evaluate medical history, current conditions, and medication regimens for health plan members
  • Educate patients and families regarding diagnosed conditions and prescribed medications
  • Identify potential care gaps and provide actionable recommendations for improving patient outcomes
  • Complete all visit documentation; no need to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited institution
  • At least 6 months of current NP experience (preferred)
  • Skilled in assessing, educating, and communicating with adult and geriatric populations
