Seeking a skilled and compassionate Nurse Practitioner to conduct home health risk assessments on a per diem basis in Benton County, WA, including areas such as Kennewick, Richland, and West Richland. This rewarding position focuses on evaluating Medicare Advantage plan members in their homes, with each assessment averaging about 45 minutes. Responsibilities include conducting either annual health assessments or post-discharge visits to support the ongoing well-being of patients in the community.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Meet minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage health plan members
• Review and assess prior/current medical histories, ongoing medical conditions, and medication use
• Offer patient education regarding diagnoses and prescribed medications
• Identify potential gaps in care and recommend appropriate next steps for improved outcomes
• No requirement to establish treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Washington State
• Current CPR certification (BLS required)
• Board certification through AANP or ANCC
• Minimum of 6 months experience as a Nurse Practitioner preferred
• Masters degree (MSN) from an accredited nursing program
