Job Description

Experienced Nurse Practitioners seeking per diem opportunities in home health will find this role both rewarding and flexible. This position involves conducting home health risk assessments for Medicare Advantage members throughout Scioto County, Ohio, including Portsmouth, Wheelersburg, New Boston, and Minford. Your assessments will be vital in promoting holistic, preventative care for members in their homes, supporting their overall health and wellness.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to choose workdays that fit your needs
  • Set your own minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Complete comprehensive annual risk assessments in members homes (average visit: 45 minutes)
  • Review previous and current medical history and medication use
  • Deliver education to members about their medical conditions and proper medication management
  • Detect gaps in care and make recommendations for resolution
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS required)
  • Master of Science in Nursing (MSN) from an accredited program
  • At least 6 months of recent experience as a Nurse Practitioner preferred
