Advance your career as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, youll provide valuable care to Medicare Advantage plan members residing throughout Guernsey County, including Cambridge Township, Cambridge, and Jackson Township. Essential Responsibilities:
- Perform comprehensive annual health risk assessments in patients homes
- Review medical and medication history, current conditions, and prior medical care
- Deliver education to patients on their medical conditions and treatment adherence
- Detect gaps in care, providing recommendations and resources for improved health outcomes
- No requirement to create detailed treatment plans or prescribe medications
Candidate Qualifications:
- Active Nurse Practitioner (NP) License in Ohio
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- Minimum of six months of recent experience as a Nurse Practitioner (preferred)
- MSN degree from an accredited nursing program
For each evaluation completed, youll receive compensation between $80$90, plus a stipend for mileage and administrative activities.
