Make a meaningful impact on patient well-being as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members in Fountain County, IN. This per diem opportunity offers the flexibility to set your own schedule, with shifts available any day of the week. Youll have autonomy in creating a workload that fits your availability, with a minimum weekly or bi-weekly hourly commitment required.

Schedule:

• Flexible shiftschoose between Monday and Sunday

• Commitment of minimum weekly or bi-weekly hours

Primary Duties:

• Perform in-home annual and post-discharge health risk assessments, averaging 45 minutes per visit
• Review and evaluate both prior and current medical history, including medications and ongoing health conditions
• Educate patients on health topics and medication usage
• Identify care gaps and provide targeted recommendations for improvement
• No treatment planning or prescribing necessaryfocus on assessment and recommendations

Preferred Qualifications:

• Active Nurse Practitioner license in Indiana
• Board certification through AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
• MSN degree from an accredited institution
• At least six months of recent NP experience preferred
