Discover a rewarding per diem opportunity to deliver patient-centered care in the field of home health. As a Nurse Practitioner specializing in home health assessments, you will be traveling to various locations within Pike County, conducting comprehensive risk assessments for Medicare Advantage plan members. The typical assessment takes approximately 45 minutes and ranges from annual evaluations to post-discharge visits, ensuring a continuum of quality care for your patients.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Ability to create your own schedule
- Commitment to a minimum weekly or bi-weekly number of hours (as you choose)
Key Responsibilities:
- Perform annual home health risk assessments within Pike County, including Glenwood, Murfreesboro, and Delight
- Evaluate patients current and past medical histories and conditions
- Review and educate patients on their medication regimens and overall health management
- Identify gaps in care and provide actionable recommendations to improve patient outcomes
- Focus solely on assessmentsno prescribing or treatment plans required
Qualifications:
- Active Nurse Practitioner License in Arkansas
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Minimum of 6 months recent NP experience preferred
- MSN from an accredited nursing program
This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
