Advance your career as a Nurse Practitioner specializing in home health by conducting comprehensive risk assessments for Medicare Advantage plan members throughout Hartford County, CT. This per diem opportunity offers unparalleled flexibility, allowing you to tailor your schedule with shifts available any day of the weekfrom Monday to Sundayand to choose your commitment on a weekly or bi-weekly basis.
Schedule/Hours:
- Flexible shifts offered Monday through Sunday
- Design your own schedule
- Set your preferred weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual health risk assessments in patients homes, each averaging around 45 minutes.
- Assess and document patients prior and existing medical histories, current medical conditions, and medication regimens.
- Educate members about their health conditions, medications, and preventive care.
- Detect care gaps and provide actionable recommendations to bridge those gaps.
- Visits may include initial annual health assessments or post-discharge check-ins.
- There is no requirement to develop treatment plans or prescribe medications.
Qualifications:
- Active Nurse Practitioner (NP) license in Connecticut.
- Board Certification through AANP or ANCC.
- 6 months or more of NP experience preferred.
- Master of Science in Nursing (MSN) from an accredited institution.
- Current CPR certification (BLS).
