Advance your clinical expertise in a flexible setting as a Nurse Practitioner specializing in home health risk assessments. This per diem role offers autonomy and variety, with each assessment supporting Medicare Advantage members across Dearborn County. Visits may include annual health evaluations or post-discharge assessments, and typically take about 45 minutes. Shifts are available in locations such as Lawrenceburg, Greendale, and Aurora, and practitioners may be scheduled in any of these areas during individual shifts.
Schedule:
- Flexible shifts offered any day, Monday through Sunday
- Design and manage your own work schedule
- Set your minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct in-home risk assessments for Medicare Advantage plan members
- Evaluate comprehensive medical histories, current conditions, and medication use
- Educate patients on health conditions and prescribed medications
- Identify care gaps and recommend solutions to improve patient outcomes
- No requirement for prescribing medications or creating ongoing treatment plans
Qualifications:
- Current Nurse Practitioner (NP) license in Indiana
- Active BLS/CPR certification
- Board Certification from AANP or ANCC
- Six months or more of NP experience preferred
- MSN degree from an accredited institution
