Job Description

Deliver comprehensive home health assessments for Medicare Advantage members by joining a per diem opportunity where your expertise as a Nurse Practitioner makes a real difference in patient outcomes. As an advanced practice provider, you will conduct in-home risk assessments across Polk County, including areas such as Urbandale, Johnston, and Clive.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate current and past medical histories, existing conditions, and medication usage
• Educate members about their medical conditions and medication regimens
• Identify gaps in care and recommend solutions to address them
• Focus on assessment and educationno treatment planning or prescribing required

Qualifications:

• Active Nurse Practitioner license in Iowa
• Current BLS (CPR) certification
• Board Certified by AANP or ANCC
• Minimum of six months current nurse practitioner experience preferred
• MSN degree from an accredited nursing program

This role offers competitive earnings, with $80$90 provided for each completed evaluation, along with a mileage stipend and administrative time compensation.
