Make a meaningful impact by delivering quality care directly to patients in their homes as a Nurse Practitioner specializing in home health risk assessments. This per diem position offers exceptional flexibility and the opportunity to serve the community throughout Pottawattamie County, including areas such as Council Bluffs, Carter Lake, and Glenwood. You will conduct home health risk assessments for Medicare Advantage plan members, focusing on comprehensive patient evaluations and education, without responsibilities for developing treatment plans or prescribing medications.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments; each visit takes approximately 45 minutes
• Evaluate patients prior and current medical history, medical conditions, and medication history
• Educate members on their health conditions and medications to promote better understanding and compliance
• Identify gaps in care, and offer recommendations to enhance continuity and quality of care
• Visits may include annual health assessments or post-discharge follow-ups

Qualifications:

• Active Nurse Practitioner license in Iowa
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited institution
• At least 6 months of experience as a Nurse Practitioner preferred
