Make a meaningful impact on patient lives by conducting home health risk assessments as a Nurse Practitioner specializing in home health services. In this dynamic, per diem role, youll support Medicare Advantage members by visiting them in their homes across various locations in Polk County, including Urbandale, Johnston, and Clive. Each assessment averages about 45 minutes and may include annual health check-ins or post-discharge follow-ups.
Schedule:
- Flexible shifts available Monday through Sunday
- Autonomy to design your own schedule
- Satisfy a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Review and assess medical history, current health conditions, and medication lists
- Deliver educational guidance on health conditions and medication management
- Identify gaps in care and make recommendations to support effective care coordination
- Note: No prescribing of medications or creation of treatment plans required
Qualifications:
- Active and unrestricted Nurse Practitioner license in Iowa
- Board Certification by AANP or ANCC required
- Current BLS (CPR) certification
- Minimum of 6 months recent experience in a Nurse Practitioner role preferred
- Master of Science in Nursing (MSN) from an accredited institution
