This role offers the unique experience of working with Medicare Advantage plan members throughout Allegany County, including Wellsville and such areas as Alfred, Cuba, and Caneadea. As part of this important healthcare initiative, you'll provide in-home evaluations to help improve health outcomes while benefiting from a highly adaptable schedule.

Schedule / Hours:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage participants
• Assess prior and current medical history, ongoing medical conditions, and medication usage
• Educate patients regarding their conditions and prescribed medications
• Detect care gaps and offer recommendations for optimal health outcomes
• No requirement for creating treatment plans or prescribing medications

Qualifications:

• Valid and active Nurse Practitioner license in New York State
• Current CPR certification (BLS)
• Board certification via AANP or ANCC
• Graduate of an accredited nursing program with an MSN degree
• Minimum 6 months of recent nurse practitioner experience preferred
