Experience a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. This flexible role focuses on providing comprehensive in-home evaluations for Medicare Advantage plan members within Allegany County, NY. Your expertise will help ensure patients in Wellsville and neighboring areas such as Alfred, Cuba, and Caneadea receive detailed health risk assessments, promoting quality care and improved health outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and assess patients medical history, current health conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and provide recommendations to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Current BLS (CPR) certification
- Board certification as a Nurse Practitioner through AANP or ANCC
- Minimum of 6 months current NP experience preferred
- MSN degree from an accredited nursing school
