Deliver high-quality, patient-centered care as a Nurse Practitioner specializing in home health risk assessments. In this per diem position, you'll have the autonomy to make a meaningful impact by conducting comprehensive health evaluations with individuals in their own homes throughout the Wellsville and broader Allegany County area. You'll interact with Medicare Advantage plan members, performing focused assessments that help bridge gaps in care and ensure optimal patient outcomes.
Schedule:
- Flexible shifts are available Monday through Sunday
- Set your own hours to best suit your lifestyle
- Commit to the minimum number of required hours weekly or bi-weekly
Key Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and assess prior and current medical history, ongoing conditions, and medication use
- Educate patients regarding their diagnoses and prescribed medications
- Identify gaps in preventive or ongoing care, recommending appropriate next steps
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
This role offers compensation of $80-$90 per completed evaluation, along with a mileage stipend and paid administrative time.
