Deliver vital healthcare services directly to patients by performing home health risk assessments in Audrain County, MO. In this per diem opportunity, youll play an important role in supporting the well-being of Medicare Advantage plan members while maintaining autonomy over your schedule. Assignments may include annual health assessments or post-discharge visits, typically lasting about 45 minutes each. You will travel within communities such as Mexico, Vandalia, and Laddonia, with flexibility in shift location and timing.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Commit to a minimum weekly or bi-weekly hourly requirement

Key Responsibilities:

• Complete annual risk assessments for Medicare Advantage members in their homes
• Review patients medical and medication history and current conditions
• Offer education on health conditions and medication management
• Identify care gaps and suggest practical solutions
• No responsibilities for creating treatment plans or prescribing

Qualifications:

• Active Nurse Practitioner license in Missouri
• Board Certification through AANP or ANCC
• Minimum of 6 months of recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
• Current CPR (BLS) certification
