Job Description
Experienced nurse practitioners are needed to perform home health risk assessments throughout Lincoln County, OK, including areas such as Chandler, Stroud, and Prague. This per diem opportunity allows you to provide valuable healthcare services to Medicare Advantage members right in their homes. Each assessment takes approximately 45 minutes and may involve annual wellness checks or post-discharge visits tailored to the individuals needs.
Schedule:
- Flexible shifts available seven days a week
- Design your own work schedule
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients previous and current medical history, ongoing conditions, and medication use
- Deliver patient education regarding diagnosed conditions and medication compliance
- Identify any gaps in patient care and offer actionable recommendations to address those needs
- Focus solely on assessment with no requirement for creating treatment plans or prescribing medications
Preferred Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current CPR certification (BLS required)
- Board certification with AANP or ANCC
- At least six months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This role offers compensation of $80$90 for each evaluation, along with a mileage stipend and administrative compensation.
