Advance your Nurse Practitioner career in a dynamic home health setting where flexibility and autonomy are prioritized. This per diem opportunity focuses on delivering comprehensive home health risk assessments for Medicare Advantage plan members throughout Payette County. Youll make a meaningful impact by helping to identify gaps in care and contributing to improved wellness for individuals in Payette, Fruitland, New Plymouth, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments (approximately 45 minutes per visit)
- Review prior and current medical history, conditions, and medications
- Offer education to patients on managing their medical conditions and medications
- Identify and report care gaps, providing recommendations to address them
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Idaho
- Current CPR/BLS certification
- Board Certification via AANP or ANCC
- At least 6 months of recent NP experience strongly preferred
- Masters degree in Nursing (MSN) from an accredited institution
Compensation for this role includes $80-$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
