Job Description

This per diem opportunity focuses on delivering comprehensive home health risk assessments for Medicare Advantage plan members throughout Payette County. Youll make a meaningful impact by helping to identify gaps in care and contributing to improved wellness for individuals in Payette, Fruitland, New Plymouth, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments (approximately 45 minutes per visit)
  • Review prior and current medical history, conditions, and medications
  • Offer education to patients on managing their medical conditions and medications
  • Identify and report care gaps, providing recommendations to address them
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current CPR/BLS certification
  • Board Certification via AANP or ANCC
  • At least 6 months of recent NP experience strongly preferred
  • Masters degree in Nursing (MSN) from an accredited institution

Compensation for this role includes $80-$90 per completed evaluation, along with a mileage stipend and pay for administrative time.
