Job Description

An engaging opportunity is available for a Nurse Practitioner experienced in home health to deliver risk assessments for Medicare Advantage members across Madison County, NC. This per diem position provides flexibility and autonomy, allowing you to manage your own schedule and impactful patient visits in communities including Mars Hill, Marshall, and Hot Springs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose and build your own schedule
  • Commitment can be weekly or bi-weekly, offering adaptable work-life balance

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate patients previous and current medical histories, chronic conditions, and medications
  • Educate patients about their health conditions and medications to promote self-management
  • Identify gaps in care and provide actionable recommendations for achieving optimal outcomes
  • Perform post-discharge and annual assessment visits as scheduled (each assessment taking approximately 45 minutes)
  • No requirements for treatment plan creation or medication prescribing

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certified through AANP or ANCC
  • Valid and current CPR (BLS) certification
  • Master of Science in Nursing (MSN) from an accredited nursing program
  • Minimum 6 months of recent experience as a Nurse Practitioner (preferred)

You will receive $80 to $90 per evaluation completed, with additional stipends provided for mileage and administrative time.
