Job Description

Deliver quality patient care in a flexible, community-focused setting as a Nurse Practitioner specializing in Home Health. This per diem opportunity supports Medicare Advantage plan members throughout Buncombe County, conducting home-based risk assessments that directly impact patient wellness and care continuity. Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly commitment required

Key Responsibilities:

  • Perform in-home annual health risk assessments with an average duration of 45 minutes
  • Review and evaluate prior/current medical history, ongoing medical conditions, and complete medication reconciliation
  • Deliver clear patient education regarding healthcare conditions and prescribed drug regimens
  • Identify care gaps and offer actionable recommendations to optimize patient outcomes
  • Visits may include annual assessments or post-discharge check-ins; no treatment planning or medication prescribing involved

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification (AANP or ANCC)
  • Masters degree in Nursing (MSN) from an accredited institution
  • Current CPR/BLS certification
  • A minimum of six months Nurse Practitioner experience is preferred
