Deliver quality patient care in a flexible, community-focused setting as a Nurse Practitioner specializing in Home Health. This per diem opportunity supports Medicare Advantage plan members throughout Buncombe County, conducting home-based risk assessments that directly impact patient wellness and care continuity. Enjoy a meaningful patient-provider experience with the freedom to create your own schedule and serve diverse populations in areas including Asheville, Montreat, Biltmore, Woodfin, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform in-home annual health risk assessments with an average duration of 45 minutes
- Review and evaluate prior/current medical history, ongoing medical conditions, and complete medication reconciliation
- Deliver clear patient education regarding healthcare conditions and prescribed drug regimens
- Identify care gaps and offer actionable recommendations to optimize patient outcomes
- Visits may include annual assessments or post-discharge check-ins; no treatment planning or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification (AANP or ANCC)
- Masters degree in Nursing (MSN) from an accredited institution
- Current CPR/BLS certification
- A minimum of six months Nurse Practitioner experience is preferred
