This per diem opportunity centers on conducting comprehensive assessments for Medicare Advantage plan members in Weatherford, OK and throughout Custer County, with shifts that can be tailored to your availability.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours and create a schedule that fits your lifestyle
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patients prior and current medical history, conditions, and medication use
- Educate patients about their health status and medication regimen
- Identify care gaps and make recommendations to improve health outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in OK
- Current CPR (BLS) certification
- Board certification (AANP or ANCC)
- Minimum of 6 months' recent Nurse Practitioner experience (preferred)
- MSN from an accredited nursing program
