Schedule:
- Flexible shifts available any day, Monday through Sunday
- Choose your hours and design your schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments during home visits, typically lasting 45 minutes each
- Review prior and current medical history, evaluate medical conditions, and assess medication histories
- Educate patients about their medical conditions and prescribed treatments or medications
- Identify gaps in care and deliver actionable recommendations to improve health outcomes
- Documentation and assessments onlyno prescribing or treatment plans required
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Current BLS (CPR) certification
- Board certification via AANP or ANCC
- Six months or more of current experience as an NP (preferred)
- MSN or higher from an accredited nursing program
In addition to a competitive rate of $80$90 per evaluation, you will receive a mileage stipend for travel within Custer County and compensation for administrative time.
