Make a meaningful impact by delivering comprehensive home health risk assessments to individuals in Tioga County, NY. As a skilled Nurse Practitioner specializing in home health, you will play an essential role in evaluating the well-being of Medicare Advantage plan members in communities such as Waverly, Owego, and Newark Valley.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum hourly commitment set weekly or bi-weekly
Key Responsibilities:
- Perform annual risk assessments in patients' homes, averaging 45 minutes per visit
- Assess patients health status through review of medical history, current conditions, and medication history
- Educate patients and caregivers on disease management and medication adherence
- Pinpoint gaps in care, promoting actionable recommendations to address health needs
- Visits may include annual health assessments or post-discharge follow-ups
- No treatment plans or prescribing responsibilities required
Qualifications:
- Current Nurse Practitioner license in NY
- Valid CPR certification (BLS)
- Active board certification via AANP or ANCC
- Graduation with an MSN from an accredited program
- At least 6 months of current NP experience preferred
