Job Description

Make a meaningful impact by delivering comprehensive home health risk assessments to individuals in Tioga County, NY. As a skilled Nurse Practitioner specializing in home health, you will play an essential role in evaluating the well-being of Medicare Advantage plan members in communities such as Waverly, Owego, and Newark Valley.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum hourly commitment set weekly or bi-weekly

Key Responsibilities:

  • Perform annual risk assessments in patients' homes, averaging 45 minutes per visit
  • Assess patients health status through review of medical history, current conditions, and medication history
  • Educate patients and caregivers on disease management and medication adherence
  • Pinpoint gaps in care, promoting actionable recommendations to address health needs
  • Visits may include annual health assessments or post-discharge follow-ups
  • No treatment plans or prescribing responsibilities required

Qualifications:

  • Current Nurse Practitioner license in NY
  • Valid CPR certification (BLS)
  • Active board certification via AANP or ANCC
  • Graduation with an MSN from an accredited program
  • At least 6 months of current NP experience preferred
