Job Description

Deliver valuable care and make a difference as a Nurse Practitioner conducting home health risk assessments within Tioga County. In this per diem role focused on Medicare Advantage members, youll perform comprehensive evaluations in client homes, providing essential insights to help improve patient outcomes. Each assessment takes approximately 45 minutes and may include annual wellness checks or post-discharge visits. Locations may vary, with visits scheduled in areas such as Waverly, Owego, and Newark Valley.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Freedom to build your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct in-home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior/current medical history, existing conditions, and medication histories
  • Offer patient education related to health conditions and medications
  • Identify gaps in care, providing recommendations to close those gaps
  • No requirement to prescribe medication or create treatment plans

Desired Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board certified through AANP or ANCC
  • Current BLS (CPR) certification
  • Masters degree in Nursing (MSN) from an accredited school
  • At least 6 months of recent NP experience (preferred)
