Job Description

Enjoy the flexibility of per diem work while making a meaningful impact in the community as a Nurse Practitioner specializing in home health risk assessments. In this role, youll conduct comprehensive, in-home evaluations for Medicare Advantage plan members across Chemung County, including areas such as Horeseheads, Elmira Heights, and Wellsburg. Each assessment takes approximately 45 minutes and may focus on an annual health review or a post-discharge evaluation, depending on the members needs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage plan members
  • Assess patient medical histories, current conditions, and medication usage
  • Educate members about their health conditions and medications
  • Identify gaps in care and provide appropriate recommendations
  • No need to develop treatment plans or prescribe medication

Qualifications:

  • Valid Nurse Practitioner license in NY
  • Masters degree in nursing from an accredited school (MSN required)
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience preferred

You will receive competitive compensation of $80$90 for each evaluation, in addition to a mileage stipend and administrative time.
