Make a direct impact on patient care through in-home health risk assessments as a skilled Nurse Practitioner. In this per diem opportunity, you will work independently within Chemung County, conducting comprehensive home health risk assessments for Medicare Advantage plan members. Flexible shifts are available throughout the week, allowing you to tailor your schedule according to your preferences, with assignments in locations such as Elmira, Horeseheads, Elmira Heights, and Wellsburg.
Schedule:
- Flexible schedulingchoose shifts between Monday and Sunday
- Set your own hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review medical and medication histories, as well as current medical conditions
- Educate patients about their health conditions, medications, and preventive strategies
- Identify care gaps and provide actionable recommendations
- Documentation onlyno need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent nurse practitioner experience preferred
