Seeking an experienced Nurse Practitioner with a background in home health to perform risk assessments for Medicare Advantage members throughout Polk County, IA. This per diem opportunity offers significant flexibility, allowing you to create a schedule that fits your lifestyle while making a real impact in patients homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage plan members in areas such as Urbandale, Johnston, and Clive
• Review prior and current medical histories, existing conditions, and medication use
• Provide patient education focusing on medical conditions and medication management
• Identify care gaps and make targeted recommendations to support patient well-being and plan compliance
• No obligation to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Iowa
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• Minimum of 6 months recent NP experience preferred
• Master of Science in Nursing from an accredited program
