Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: arrange shifts to fit your availability
• Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

• Conduct thorough annual risk assessments and post-discharge visits (approx. 45 minutes per evaluation) in members homes within Council Bluffs, Carter Lake, Glenwood, and surrounding areas
• Evaluate members medical histories, current health conditions, and medication use
• Educate patients about their medical conditions and prescribed medications
• Identify gaps in care and recommend practical steps for resolution to enhance health outcomes
• No treatment planning or medication prescribing required

Qualifications:

• Current and active Nurse Practitioner license (IA)
• Certification in Basic Life Support (BLS)
• Board certification through AANP or ANCC
• Six months or more of Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) degree from an accredited program
