The focus is on comprehensive yet concise evaluations, typically taking about 45 minutes per visit, and may include either annual assessments or post-discharge visits based on patient needs.
Schedule:
- Flexible shifts available Monday through Sunday

Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Assess prior and current health history, including conditions and medication usage
- Deliver patient education about ongoing medical conditions and current medications
- Identify and document care gaps, recommending actions to address them
- No requirement to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Iowa (IA)
- Current CPR certification (BLS required)
- Board certification through AANP or ANCC
- Minimum of six months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited program
