Schedule:

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Assess prior and current health history, including conditions and medication usage
  • Deliver patient education about ongoing medical conditions and current medications
  • Identify and document care gaps, recommending actions to address them
  • No requirement to create treatment plans or prescribe medication

Qualifications:

  • Active Nurse Practitioner license in Iowa (IA)
  • Current CPR certification (BLS required)
  • Board certification through AANP or ANCC
  • Minimum of six months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited program
