Deliver impactful care in a dynamic per diem role providing home health risk assessments for Medicare Advantage plan members throughout Jefferson County. As a Nurse Practitioner specializing in home health, you will perform thorough in-home evaluationsspanning annual health assessments and post-discharge visitswith each assessment taking about 45 minutes. Shifts may be scheduled in various communities within Jefferson County, including Watertown, Adams, Fort Drum, and Le Ray.

Schedule:

• Flexible scheduling available Monday through Sunday
• Self-create your own work schedule
• Option to select a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage plan participants.
• Assess prior and current medical conditions, history, and medications during each patient visit.
• Educate members about their health conditions and the medications they are prescribed.
• Recognize and address any gaps in care, providing recommendations to resolve identified issues.
• Note: No requirements for designing treatment plans or prescribing medications.

Qualifications:

• Current Nurse Practitioner license in New York
• Active BLS/CPR certification
• Board Certification through AANP or ANCC
• Minimum of six months of recent Nurse Practitioner experience preferred
• MSN from an accredited nursing institution
