Step into a dynamic per diem opportunity designed for experienced Nurse Practitioners passionate about home health. This role involves conducting comprehensive risk assessments for Medicare Advantage plan members across Jefferson County, NY. Your visitstypically lasting 45 minutesspan annual health checkups, as well as post-discharge assessments, adapted to the needs of each individual. Assignments may be scheduled in Watertown, Adams, Fort Drum, Le Ray, and other areas throughout the county.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete in-home health risk assessments for Medicare Advantage participants
- Review patient history, current conditions, and medication usage
- Deliver patient education related to illness management and prescription regimens
- Recognize and address care gaps with constructive recommendations
- No requirements to create ongoing treatment plans or prescribe medication
Qualifications:
- Unrestricted New York State Nurse Practitioner license
- Active BLS CPR certification
- Board Certified by AANP or ANCC
- Minimum of 6 months current NP experience preferred
- MSN from an accredited nursing program
This position offers compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and administrative time for documentation and follow-up.
