Job Description

Make a meaningful impact on patient care through home health risk assessments in Cayuga County, NY. This per diem opportunity is ideal for Nurse Practitioners interested in flexible, self-directed work and community-based care. Youll visit Medicare Advantage members in their homes, conducting 45-minute evaluations focused on health histories and patient educationwithout the responsibility of creating treatment plans or prescribing medications.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly commitment as preferred

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members across Cayuga County, including Auburn, Cayuga, Union Springs, and Moravia
  • Review and evaluate prior and current medical histories, ongoing conditions, and medication use
  • Educate patients about health conditions and safe, effective medication management
  • Identify potential care gaps and recommend actionable next steps
  • Collaborate with the broader healthcare team to support continuity of care

Qualifications:

  • Active Nurse Practitioner license in NY
  • Current CPR certification (BLS)
  • Board-certified by AANP or ANCC
  • At least 6 months experience working as a Nurse Practitioner is preferred
  • Masters in Nursing (MSN) from an accredited institution

Compensation for this unique, community-focused role is $80$90 per evaluation, with additional mileage reimbursement and administrative pay.
