Take your expertise on the road as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Monroe County, Illinois. This per diem opportunity offers true flexibility and independenceperfect for advanced practice clinicians seeking a dynamic, patient-centered environment. You will conduct comprehensive evaluations in the homes of residents in communities like Waterloo, Columbia, and Valmeyer. Select your preferred shifts, decide your own schedule, and make a measurable difference in preventative care.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical history, medical conditions, and medication history
- Educate members on their health conditions and prescribed medications
- Identify care gaps and provide recommendations without managing ongoing treatment or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Illinois
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of Nurse Practitioner experience preferred
