Deliver meaningful patient care in a rewarding per diem role focused on home health risk assessments for Medicare Advantage plan members. As a Nurse Practitioner specializing in home health, you will complete comprehensive evaluations in residences across Erie County, tailoring your schedule to fit your life with flexible shifts and self-selected hours between Monday and Sunday. Enjoy the autonomy of creating your own schedule and the opportunity to serve a diverse community, including the areas of Erie, Millcreek, and Corry.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: design your own work hours
- Minimum hourly commitment on a weekly or bi-weekly basis
Responsibilities:
- Conduct annual and post-discharge home health risk assessments, each lasting approximately 45 minutes
- Review and document prior and current medical histories, chronic medical conditions, and medication lists
- Educate members on their medical conditions and medication regimens
- Identify care gaps and recommend appropriate actions to address them
- No requirement to prescribe, treat, or develop care plans
Qualifications:
- Active Nurse Practitioner license in Pennsylvania
- Valid CPR certification (BLS)
- Certification through AANP or ANCC
- Minimum of six months recent NP experience preferred
- Master of Science in Nursing (MSN) degree from an accredited institution
