Job Description

Experienced Nurse Practitioners are needed to deliver vital home health risk assessments across New Haven County, CT, including locations such as Waterbury, New Haven, and Hamden. This per diem opportunity offers exceptional flexibility, allowing you to build your own schedule and work shifts that align with your personal and professional commitments.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Option to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage plan members in their residences
  • Perform evaluations of prior and current medical history, as well as medical conditions and medication use
  • Deliver patient education on health conditions and medication management
  • Identify any gaps in care and provide recommendations to promote improved health outcomes
  • Visits may include annual assessments or post-discharge follow-ups
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Board Certification through AANP or ANCC
  • Up-to-date CPR certification (BLS)
  • MSN degree from an accredited institution
  • At least 6 months of current nurse practitioner experience is preferred
