As a Nurse Practitioner specializing in home health, you'll play a vital role in patient well-beingconducting thorough risk assessments, offering valuable health education, and helping to bridge gaps in care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Control your own schedule
  • Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive annual risk assessments for Medicare Advantage members in home settings
  • Review patients medical histories, current health status, and medication regimens
  • Deliver health education regarding conditions and prescriptions directly to patients
  • Identify care opportunities and recommend next steps, ensuring continuity and quality of care
  • Maintain documentation following established guidelines
  • No treatment planning or prescribing required

Qualifications:

  • Active Nurse Practitioner license in Connecticut (CT)
  • Board Certified through AANP or ANCC
  • Current CPR certification (BLS required)
  • Graduate of an accredited MSN program
  • Minimum of 6 months Nurse Practitioner experience preferred
  • Strong clinical judgment and interpersonal communication skills
