A rewarding opportunity is available for experienced Nurse Practitioners specializing in Home Health to provide home-based risk assessments for Medicare Advantage plan members in Pitt County, North Carolina. This per diem role offers the autonomy to design your own schedule, with flexible shifts between Monday and Sunday and the ability to determine your minimum weekly or bi-weekly hour commitment.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to suit your needs
- Commit to minimum coverage on a weekly or bi-weekly basis
Primary Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage patients within Pitt County (areas such as Greenville, Winterville, and Ayden)
- Review and evaluate prior/current medical and medication history
- Provide clear education to patients regarding medical conditions and prescribed medications
- Identify care gaps and recommend appropriate next steps
- Each risk assessment typically lasts about 45 minutes
- No need to prescribe medications or create ongoing treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current CPR certificate (BLS)
- MSN degree from an accredited nursing school
- At least 6 months of current NP experience preferred
Compensation:
- Earn $80-$90 per completed risk assessment, plus a mileage stipend and payment for administrative time
