This role centers on delivering patient-centered care for Medicare Advantage plan members across Peoria County, Illinois, including locations such as Peoria, Chillicothe, and Bartonville.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual home health risk assessments in clients' homes
• Complete thorough evaluations of patients medical history, current conditions, and medication use
• Educate patients on chronic conditions and proper medication management
• Identify care gaps and suggest strategies to resolve them during visits
• Visits may include annual health assessments or post-discharge follow-ups
• No treatment plan development or prescribing required

What Were Looking For:

• Active Nurse Practitioner license in Illinois
• MSN from an accredited nursing school
• Certification through AANP or ANCC
• Current BLS certification (CPR)
• Minimum of 6 months experience as a Nurse Practitioner preferred
• Excellent communication and documentation skills
• Strong critical thinking with the ability to work independently

This position offers compensation of $80-$90 per completed evaluation, as well as a mileage stipend and allowance for administrative time, recognizing the value of your expertise and commitment to quality care.
