Job Description

Enjoy a dynamic per diem opportunity as a Nurse Practitioner focused on home health risk assessments. This role centers on providing high-quality, in-home risk assessments to Medicare Advantage members throughout Peoria County, Illinois, covering areas such as Peoria, Chillicothe, and Bartonville. Each assessment typically lasts about 45 minutes and could include annual wellness checks or post-discharge visits. You will have the flexibility to create your own schedule, choosing shifts between Monday and Sunday, with a minimum weekly or bi-weekly commitment.

Schedule:

Essential Responsibilities:

  • Complete annual risk assessments for Medicare Advantage members in their homes
  • Review prior and current medical history, conditions, and medications
  • Educate patients regarding their diagnoses and medications
  • Identify and document care gaps, recommending follow-up when necessary
  • No involvement in creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Current BLS (CPR) certification
  • Board certification via AANP or ANCC
  • At least 6 months of recent NP experience preferred
  • MSN from an accredited nursing program
