Step into a dynamic Nurse Practitioner role focused on home health risk assessments in Fayette County, Ohio. This per diem position empowers Nurse Practitioners to make a significant impact on patient care by meeting Medicare Advantage members in their homes throughout Washington Court House and surrounding communities such as Jeffersonville, Bloomingburg, and Milledgeville.

Main Responsibilities
- Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate prior and current medical history, existing medical conditions, and medication use
- Educate patients on their health conditions and prescribed therapies
- Identify gaps in care and deliver targeted recommendations to enhance patient outcomes
- No requirement to create treatment plans or prescribe medications
Required Qualifications
- Active Nurse Practitioner license in Ohio
- Board certification through AANP or ANCC
- Valid MSN degree from an accredited nursing program
- Current BLS (CPR) certification
- At least 6 months of Nurse Practitioner experience is strongly preferred
