Job Description
Shape patient care in Duplin County, NC, through impactful and independent home health risk assessments. As a Nurse Practitioner specializing in home health, you will help improve health outcomes for Medicare Advantage members by performing evaluations directly in patients homes. Youll gain the flexibility and autonomy of a per diem opportunity while making a meaningful difference in Wallace, Mount Olive, Warsaw, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum hourly requirement on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive home health risk assessments, averaging 45 minutes per visit
- Review and evaluate patients past and current medical histories, health conditions, and medication usage
- Educate patients on their medical conditions and prescribed medication regimens
- Identify care gaps and offer actionable recommendations to address them
- Focus exclusively on assessmentno need to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR/BLS certification
- Board certification through AANP or ANCC
- At least 6 months of experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
