Deliver high-quality patient care by performing comprehensive home health risk assessments for Medicare Advantage members across Duplin County. In this per diem Nurse Practitioner position, youll have the autonomy to create your own schedule, selecting flexible shifts from Monday through Sunday and meeting a minimum weekly or bi-weekly hourly commitment. Your primary focus will be to complete thorough risk assessmentseach lasting approximately 45 minutesat patient residences in communities including Wallace, Mount Olive, and Warsaw.

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Review patients prior and current medical histories, conditions, and medications
• Provide personalized education on health conditions and medication management
• Identify care gaps and recommend actionable solutions to enhance patient health outcomes
• No responsibility to prescribe medications or develop treatment plans

Required Qualifications:

• License as a Nurse Practitioner in North Carolina
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• Minimum of 6 months recent experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program

• Flexible shifts available MondaySunday

• Minimum hourly commitment required on a weekly or bi-weekly basis

Compensation for this role is competitive, offering $80-$90 per evaluation in addition to a mileage stipend and paid administrative time.
