Job Description
Deliver essential care and assessments to patients in their homes as a Nurse Practitioner specializing in home health risk evaluations. In this per diem position, you'll visit Medicare Advantage members throughout Duplin County, conducting thorough health risk appraisals in communities such as Wallace, Mount Olive, and Warsaw. Each assessment typically takes about 45 minutes and may be an annual review or a post-discharge evaluation.

Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and assess prior and current medical history, chronic conditions, and medication use
- Deliver patient education regarding health conditions and prescribed treatments
- Identify care gaps and offer guidance to address these needs
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- Minimum of 6 months recent experience as an NP preferred
This role offers competitive compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative tasks performed outside of visits.
