Experienced Nurse Practitioners are invited to take on a rewarding per diem opportunity focused on home health risk assessments for Medicare Advantage plan members in Benton County, MO. This flexible home-based position supports communities in Warsaw, Cole Camp, Lincoln, and White Branch. Enjoy autonomy by creating your own schedule, with shifts available every day of the week.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule to fit personal and family needs
• Choose from minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments (approximately 45 minutes each) in patient residences
• Review and document medical history, current medical conditions, and medication usage
• Educate patients on conditions and medication management during visits
• Identify gaps in care and offer recommendations for care improvement
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Missouri
• Master of Science in Nursing (MSN) from an accredited institution
• Board Certification through AANP or ANCC required
• Current CPR certification (BLS)
• Minimum of 6 months Nurse Practitioner experience preferred
