Step into a meaningful per diem opportunity as a Nurse Practitioner specializing in home health assessments throughout Warren County, NC, including the areas of Norlina, Warrenton, and Macon. In this rewarding role, youll work directly with Medicare Advantage plan members to complete detailed risk assessments in the comfort of their homes. Each assessment typically takes about 45 minutes and can range from annual check-ups to post-discharge visits. You have the flexibility to determine your own schedulechoose your hours between Monday and Sunday with a minimum weekly or bi-weekly commitment to suit your lifestyle.

Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Thoroughly review and evaluate patients medical histories, current health concerns, and medication lists
- Deliver patient education regarding medical conditions and safe medication use
- Detect and document any gaps in care, recommending next steps as needed
- Note: There is no requirement to create treatment plans or prescribe medications
Desired Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board certified (AANP or ANCC)
- A minimum of 6 months recent NP experience preferred
- Master of Science in Nursing from an accredited institution
