Advance your career and make a meaningful impact on patient outcomes as a Nurse Practitioner providing home health risk assessments on a per diem basis. In this engaging and flexible role, you will visit Medicare Advantage plan members throughout Vance County, conducting thorough assessments and offering education and guidance to support ongoing wellness and care needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commitments may be weekly or bi-weekly, based on your availability
Key Responsibilities:
- Perform comprehensive annual health risk assessments for Medicare Advantage plan members in their homes
- Assess and document medical history, current conditions, and medication regimens
- Deliver patient education regarding medical conditions and prescribed medications
- Identify care gaps and provide actionable recommendations to help address them
- Visits generally last approximately 45 minutes; no treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Current BLS/CPR certification
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
In this opportunity, youll be compensated $80-$90 for each completed evaluation, in addition to a mileage stipend and support for administrative responsibilities.
