An exciting per diem opportunity is open for Nurse Practitioners with a passion for home health assessment. This role involves conducting in-home risk assessments for Medicare Advantage plan members throughout Drew County, AR, including the communities of Monticello, Wilmar, and Winchester. Each risk assessment typically lasts around 45 minutes, and assignments may range from routine annual health check-ups to post-discharge visits. Practitioners have the chance to create their own schedules with flexible shifts available seven days a week, and only a minimal commitment of hours is required weekly or bi-weekly.
Schedule:
- Flexible shifts offered Monday through Sunday
- Self-scheduling to align with personal availability
- Minimal required weekly or bi-weekly commitment
Key Responsibilities:
- Complete annual in-home health risk assessments for Medicare Advantage members
- Review past and current medical history, along with medication regimens
- Educate patients about health conditions and medication management
- Identify care gaps and provide recommendations to help close those gaps
- No requirement to prescribe medications or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR certification (BLS)
- Board certification (AANP or ANCC)
- Minimum of 6 months of NP experience preferred
- MSN from an accredited nursing program
