Deliver vital healthcare to Medicare Advantage members by providing specialized home health risk assessments on a per diem basis. This role offers the unique flexibility to customize your schedule, with shifts available throughout the week to suit your availability. The assessments will take place in various residential settings across Drew County, AR, including Monticello, Wilmar, and Winchester, with each evaluation typically lasting around 45 minutes. Visits may involve annual health assessments or post-discharge evaluations, focusing on comprehensive risk analysis to support member well-being.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform thorough home health risk assessments for Medicare Advantage plan participants
- Review and evaluate medical history, current conditions, and medications
- Educate members on their health status, diagnoses, and prescribed medications
- Identify care gaps and provide actionable recommendations for follow-up
- No development of treatment plans or medication prescribing involved
Qualifications:
- Active Nurse Practitioner License in AR
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of current NP experience preferred
- MSN degree from an accredited nursing school
You will receive $80$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
