Step into a dynamic per diem opportunity designed for experienced Nurse Practitioners skilled in home health care. This role focuses on conducting home health risk assessments for Medicare Advantage plan members across Dutchess County, including areas such as Beacon, Fishkill, Rhinebeck, and other nearby communities. Each assessment typically lasts 45 minutes and may involve annual evaluations or post-discharge visits, allowing you to work autonomously while providing essential care insight.

Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own work schedule
• Minimum commitment per week or every two weeks based on your preference

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage health plan members
• Collect and review prior and current medical history, existing medical conditions, and medication lists
• Deliver patient education regarding ongoing medical conditions and medications
• Identify potential gaps in care and recommend actionable solutions
• Note: This role does not require treatment plan development or medication prescribing

Qualifications:

• Valid Nurse Practitioner license in New York State
• Active CPR certification (BLS)
• Board certification from AANP or ANCC
• Minimum of 6 months current NP experience preferred
• MSN degree obtained from an accredited nursing program
