Job Description

Deliver high-impact care as a Nurse Practitioner specializing in home health risk assessments throughout Dutchess County. This per diem opportunity offers you the flexibility to create your own schedule, allowing for an ideal work-life balance. Serve Medicare Advantage plan members in communities including Poughkeepsie, Beacon, Fishkill, Rhinebeck, and Wappingers Falls, conducting thorough health evaluations in their homes.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate current and past medical history, health conditions, and medications
  • Educate patients on health conditions and proper medication usage
  • Detect gaps in care and provide clinical advice or recommendations for improvement
  • Visits may include annual assessments or post-discharge evaluations
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Master's degree (MSN) from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience strongly preferred
