If you are an experienced Nurse Practitioner seeking a flexible per diem opportunity in home health, consider this role focused on risk assessments for Medicare Advantage plan members in Shelby County, Ohio. You'll play an important role in supporting patient wellness during visits in Sidney, Anna, Fort Loramie, Russia, and nearby communities.
Schedule:
- Flexible shifts, Monday through Sunday
- Set your own availability
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments, each lasting approximately 45 minutes
- Review and evaluate prior and current medical histories, ongoing medical conditions, and medication use
- Educate members on their health conditions and medication regimens
- Identify care gaps and make recommendations for follow-up or resolution
- Focus exclusively on assessment and educationno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- MSN degree from an accredited institution
- At least 6 months of Nurse Practitioner experience preferred
In this role, youll earn between $80 and $90 per evaluation, plus receive a mileage stipend and compensation for administrative time.
