Shifts are available in Orangeburg County, covering areas such as Orangeburg, Holly Hill, and North, with self-scheduling options from Monday through Sunday.

Schedule

Key Responsibilities

• Perform comprehensive home health risk assessments, averaging 45 minutes per visit
• Evaluate patients current and prior medical history, assess conditions, and review medication use
• Provide patient education regarding health conditions and medication regimens
• Identify and address gaps in care, delivering actionable recommendations for resolution
• Visits may include annual assessments or post-discharge check-ins
• No requirement to design treatment plans or prescribe medications

Qualifications

• Active Nurse Practitioner license for South Carolina
• Board Certification through AANP or ANCC required
• Valid CPR certification (BLS)
• MSN degree from an accredited nursing school
• At least 6 months of recent NP experience preferred
