Deliver quality patient care as a Nurse Practitioner by providing comprehensive home health risk assessments for Medicare Advantage plan members across Marlboro County, SCincluding Bennettsville, McColl, and Clio. This per diem opportunity allows you to utilize your expertise, make a difference in patients lives, and enjoy exceptional schedule flexibility.

Schedule Information:

• Flexible shifts available Monday through Sunday
• Create your own schedule to meet your lifestyle and preferences
• Select minimum weekly or bi-weekly hourly commitment options

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
• Evaluate medical histories, current conditions, and medication usage
• Deliver clear patient education regarding diagnoses and medications
• Detect care gaps and recommend solutions to improve patient outcomes
• No requirement for prescribing medications or creating treatment plans

Desired Qualifications:

• Active Nurse Practitioner license issued by South Carolina
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least six months of recent Nurse Practitioner experience preferred

Providers in this role are compensated $80-$90 per completed evaluation, with additional mileage stipends and administrative time included to support your efforts.
