Schedule:

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Assess prior and current medical histories, underlying health conditions, and full medication lists
  • Educate patients on their conditions and prescribed medications
  • Identify care gaps and provide actionable recommendations for improved outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of recent experience as a Nurse Practitioner preferred
  • Master of Science in Nursing from an accredited institution

This opportunity covers Wallace, Mount Olive, Warsaw, and other areas in Duplin County; you may be scheduled in any of these locations per shift. Each risk assessment typically takes about 45 minutes, with visits ranging from annual check-ins to post-discharge evaluations.
