Step into an impactful Nurse Practitioner opportunity specializing in home health assessments for Medicare Advantage members throughout Duplin County. In this per diem position, you'll conduct focused home health risk evaluationseither annual or post-dischargesupporting the health and wellbeing of residents in areas like Wallace, Mount Olive, and Warsaw. Each assessment generally takes about 45 minutes, as you visit patients in their homes and assess their ongoing healthcare needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage health plan members in residential settings
• Review prior and current medical history, evaluate present conditions, and assess medication usage
• Educate patients and caregivers regarding health conditions and medication management
• Identify care gaps and offer informed recommendations to address risks
• No need to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Valid CPR (BLS) certification
• Board Certification from AANP or ANCC
• Master's degree in Nursing (MSN) from an accredited institution
• At least 6 months of current Nurse Practitioner experience preferred

Youll receive competitive compensation of $80-$90 for each evaluation, along with a mileage stipend and additional compensation for administrative time.
