Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to design your own schedule
- Minimum weekly or bi-weekly hourly commitment as preferred
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Assess prior and current medical histories, underlying health conditions, and full medication lists
- Educate patients on their conditions and prescribed medications
- Identify care gaps and provide actionable recommendations for improved outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing from an accredited institution
This opportunity covers Wallace, Mount Olive, Warsaw, and other areas in Duplin County; you may be scheduled in any of these locations per shift. Each risk assessment typically takes about 45 minutes, with visits ranging from annual check-ins to post-discharge evaluations.
