Job Description

Advance your healthcare career as a dedicated Nurse Practitioner performing home health risk assessments for Medicare Advantage members in Shoshone County, ID. This per diem opportunity offers the unique flexibility to create your own schedule, with shifts available any day of the week between Monday and Sunday. You will have the autonomy to determine your weekly or bi-weekly commitment and may be scheduled in various locations throughout Kellogg, Pinehurst, Osburn, and the surrounding Shoshone County area.

Schedule:

  • Flexible shifts, Monday through Sunday
  • Self-scheduled; design your own work hours
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual or post-discharge home health risk assessments for Medicare Advantage members
  • Review past and current medical histories, existing medical conditions, and complete medication histories
  • Educate patients on their health conditions and medications during home visits
  • Detect gaps in patient care and provide informed recommendations for follow-up
  • This role focuses on assessments only; prescribing or treatment planning is not a requirement

Qualifications:

  • Current Nurse Practitioner (NP) license in Idaho
  • Active Board Certification through AANP or ANCC
  • Up-to-date CPR certification (BLS)
  • MSN degree from an accredited school of nursing
  • At least 6 months of recent NP experience preferred

Compensation for this position includes $80-$90 per evaluation, along with a mileage stipend and paid administrative time.
