Job Description

Seeking a Nurse Practitioner to perform home health risk assessments on a per diem basis within Columbia County, AR, including towns such as Magnolia, Waldo, and Taylor. In this flexible per diem role, you will visit Medicare Advantage plan members to deliver comprehensive health evaluations. Each home visit typically takes 45 minutes and may include annual assessments or post-discharge visits, depending on member needs. You have the autonomy to create your own schedule, with minimum weekly or bi-weekly hourly commitments.

Schedule:

  • Flexible shifts offered between Monday and Sunday
  • Self-scheduling options available

Key Responsibilities:

  • Complete annual risk assessments for Medicare Advantage plan members in their homes
  • Review and document medical history, existing conditions, and medication use
  • Educate members about their health conditions and prescribed medications
  • Identify care gaps and recommend actionable next steps
  • No requirement for developing treatment plans or writing prescriptions

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification through AANP or ANCC
  • CPR (BLS) certification must be current
  • MSN from an accredited nursing program
  • Six months of recent Nurse Practitioner experience is preferred
