Take your nurse practitioner skills on the road with this dynamic per diem opportunity centered on home health risk assessments for Medicare Advantage plan members across Muskogee County. As a provider, youll impact patients lives by conducting thorough health evaluations in their homes. Your clinical insights and educational support will help improve care, prevent hospitalizations, and close gaps impacting overall well-being.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Design your own work schedule
- Meet a minimum commitment for hours on a weekly or bi-weekly basis
What Youll Do:
- Travel to members residences within Muskogee, Fort Gibson, Warner, and other areas in Muskogee County to complete risk assessments (each visit approximately 45 minutes)
- Review and evaluate comprehensive health histories, current conditions, and medications
- Offer clear education to members about their health and medication regimens
- Identify potential care gaps and provide actionable recommendationswithout managing ongoing treatment or prescribing
- Collaborate and document findings for the healthcare plan, ensuring accurate reporting
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Board certification from AANP or ANCC
- Current CPR (BLS) certification
- Graduate of an accredited MSN program
- At least 6 months of current experience as a Nurse Practitioner preferred
This per evaluation role offers $80-$90 per completed assessment, plus a mileage stipend and compensation for required administrative time.
