Job Description

Advance your career as a Nurse Practitioner with a unique per diem opportunity delivering essential home health risk assessments. In this flexible position, youll visit Medicare Advantage plan members across Grant County, including communities such as Marion, Gas City, and Upland. Your clinical expertise will directly support patient wellness with a focus on risk assessment and care gap identificationwithout prescribing or treatment planning responsibilities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-schedulingtailor your hours to fit your life
  • Commit to a minimum weekly or bi-weekly hourly schedule

Core Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and evaluate patients current and prior medical history, chronic conditions, and medication profiles
  • Offer clear, concise education on health conditions and prescribed medications
  • Detect gaps in care, providing sound recommendations for follow-up and resolution
  • Each visit typically lasts approximately 45 minutes and may include annual or post-discharge health checks
  • No treatment plan creation or prescription duties required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • National board certification via AANP or ANCC
  • At least six months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited program
